First Aid
One small red cross between your guests and a plummeting park rating.
First Aid is a small support facility available in RollerCoaster Tycoon and its sequels, serving as the park's on-site medical station. Rather than being a thrill ride, it is a practical infrastructure building that park operators place near ride clusters and food stands to treat guests who become sick from poor-quality food or injured from the g-forces of their latest coaster drop.
In gameplay, First Aid stations directly reduce the number of sick and injured guests wandering your park, which in turn protects your park rating and keeps guests spending money rather than storming out in a foul mood. It is one of the few facilities available from the very start of a new park, making it an early and essential part of any sensible park layout.

Lore & Background
In the world of RollerCoaster Tycoon, a park is only as healthy as its guests. The moment a visitor eats a stale hot dog or survives a particularly vicious inversion on a new coaster, they turn green or clutch their ribs, and the park's reputation begins to bleed. The First Aid station exists to be the quiet, unglamorous hero that catches those guests before they reach the exit gate and file a one-star review.
Park operators in the RCT universe treat First Aid the way a real-world theme park treats its medical tent: it is not something you advertise on the front gate, but it is something you absolutely cannot afford to skip. Seasoned tycoons will cluster two or three stations near the busiest ride corridors and food courts, knowing that a single sick guest left untreated can sour an entire family's afternoon and drag the park rating down by several points.
The community has a fond, slightly affectionate relationship with the little red-cross kiosk. It is the building every new player forgets to build in their first park, watches their rating crater, and then frantically slaps down three of them while muttering, "Oh, I thought that was just decoration." In that small moment, First Aid earns its place in the pantheon of RCT's most beloved non-rides.
In Their Own Story
The afternoon sun hung low over Cedar Point's eastern ridge, painting the steel of the new Bobsled Run in shades of tangerine. Margot, the park's operations manager, watched from the back of the First Aid kiosk as a family of four shuffled toward the exit. The father was pale. The mother was cradling a child who had turned the exact shade of the hot dogs they'd eaten at the food court forty minutes ago. The teenager kept pressing a hand to his ribs, the afterglow of eight inversions still humming in his shoulders.
Margot stepped out, clipboard in one hand, and intercepted them with a practiced smile. "You're in luck. I've got ice, water, and a very forgiving stomach tablet right here." She gestured to the bench inside the kiosk, where a small red cross gleamed on the wall above a first-aid kit and a box of ginger chews.
Twenty minutes later, the family was back on the queue for the Bobsled Run, the child already demanding a second go-round. Margot logged the incident, updated her sick-guest tally, and walked back toward the ride cluster. Three more First Aid stations to inspect before closing. The park rating held at 72. For tonight, that was enough.

First Aid is a small, single-purpose support building you place on the park map to treat guests who become sick (typically from low-quality food) or injured (from ride g-forces and drops). It does not generate income directly, but it reduces the number of sick and injured guests, which protects your overall park rating and keeps visitors from leaving in disgust.
In practice, you want to position First Aid stations near clusters of rides and food stands, since that is where the sick and injured guests originate. A common strategy is to place one near the main food court and one or two near the busiest ride corridors. Because it is available from the very start of a new park and costs relatively little, there is no reason to delay building one.
Community nickname: "The Red Cross Kiosk" or, more affectionately, "The Regret Station" — the building every new player forgets to build, watches their rating tank, and then frantically slaps down three of them while apologising to their guests. It is the RCT equivalent of reading the manual after the engine won't start.
Did You Know?
- First Aid is one of the very few support buildings available with zero research, meaning a brand-new park can place one on the first day of operation.
- Sick guests (from food) and injured guests (from rides) are tracked as separate guest states, and the First Aid station helps with both, making it a dual-purpose medical facility.
- In RCT, if you neglect First Aid and let sick guests pile up, they can drag your park rating down by several points each, sometimes faster than a single bad ride review.
- The First Aid kiosk is visually one of the smallest buildable structures in the game, often just a 2×2 tile footprint, yet it can be the single most important building for keeping a large park stable.
